This session will discuss how to design, develop, and deliver integrated care models, primary care or place-based health care, through community-leadership approaches. How do we empower the community to have meaningful conversations about health and wellbeing, and to facilitate solutions that they want for their communities, and will ask the question about the role community and voluntary organisations have in representing communities.
